Exercise Management Aug 1, 2016 - Aug 31, 2017

All exercise logs must be complete by August 31*

*Health Bucks are applied to the upcoming plan year for programs completed by August 31. Health Bucks for programs completed after August 31 are applied to the following Plan Year.  NOTE:  Health Bucks can only be applied one time per calendar year per participant.

Exercise Management Requirements:
  • Must exercise at least 2.5 hours per week at a minimum of 3 days per week (and/or at least 7,500 pedometer steps at least 4 days per week). Refer to appropriate exercise types below. {An absentee rate greater than an average of 2 sessions per month will exclude participation for Health Bucks}
  • Must complete an Exercise Management Progress Report* on a monthly basis and submit reports at end of program. Must be submitted by August 31 in order for Health Bucks to be issued and effective January 1.
    * The Exercise Management Progress Report is available to use, however, other Exercise Logs are acceptable.
  • Health Bucks Reward: Forms can be submitted for Health Bucks Reward at $12/month UP TO $144 PER YEAR. For example: 4 months = $48, 5 months = $60, 6 months = $72, etc...12 months = $144
